Understanding Connecticut’s Adult Family Living Program

Who May Be Eligible?

- Are enrolled in Connecticut Medicaid (Title 19) (Medicare alone does not qualify)
- Live in Connecticut
- Require assistance with day-to-day activities
- Are able to direct their own care or have a representative who can do so
- Live in the same household as their caregiver
Who May Serve as a Caregiver
- An adult child (18+)
- Another relative
- A friend who lives in the home
- In certain circumstances, a legal spouse or legally responsible individual, subject to program rules and approval
Caregivers must live in the same residence as the individual receiving care and provide support based on an approved plan of care.
